Physical Information 2026

Good morning!

Reminder that any student wishing to participate in any sports this 206-2027 school year, MUST have a valid physical on file BEFORE participating in any try out or practice.  Physicals MUST be dated ON or AFTER April 1, 2026 to be valid for the 2026-2027 school year.  Physicals MUST be signed by a doctor or nurse practitioner along with their license number.  I have attached ALL the required forms needed to be eligible for the upcoming seasons.

GIRLS GOLF first mandatory practice will be on  Friday, July 31 and all girls participating, MUST have a physical on or before that date to continue with golf.

BOYS/GIRLS CROSS COUNTRY, FOOTBALL, VOLLEYBALL, & BOYS TENNIS first mandatory practice is on Monday, August 3 and any student wishing to participate MUST have a physical on or before this date to continue with that respective sport.

I have attached the Drug Consent Form and CIO Consent Forms along with the Sudden Cardiac Arrest and Concussion Forms.  These are needed every year and are also attached to the physical form.

The Madison County Board of Health will be holding free physicals and immunizations at the Anderson office (206 East 9th Street, Anderson) Monday-Friday, 8:30am-4pm and Tuesdays will be 8:30-7pm.  At the Elwood office (226 South Anderson Street, Elwood) on Thursdays 9:30am-3pm.  Please call 765-646-9206 to schedule an appointment at either location.
